Who we are
This website is operated for Destin Implant and Smile Studio, a dental practice in Destin, Florida.
Information collected through the questionnaire
The questionnaire asks about dental concerns, prior treatment, treatment goals and interests, payment preferences and, where relevant, credit range and possible funding options. It also collects your name, telephone number, email address, ZIP code, preferred contact method and contact time, and your appointment and marketing communication choices.
The website records a questionnaire session identifier, your progress, the page address and browser information. Please avoid including personal or medical information in website links or URL parameters. The questionnaire is not an emergency service and is not a substitute for a clinical assessment.
Answers may be saved before you finish
Your selected answers and questionnaire progress are saved as you move between questions. Contact information may also be saved when you leave the phone or email field, before pressing Continue. Incomplete questionnaires may remain in the practice's systems. Checking a communication box is not required for this saving to occur.
Why information is used
Questionnaire and contact information are used to process consultation requests, help the team discuss treatment and payment options, and contact you as authorized. The questionnaire does not make a clinical diagnosis and does not guarantee treatment or financing eligibility. Optional promotional communications are handled separately from appointment-related communications.
Systems that receive information
Questionnaire information is saved using our website's managed cloud database. Completed submissions are sent from the website's server to Dental LeadFlow, the practice's customer relationship management system, over HTTPS. The CRM receives your contact details, the survey answers shown to you, and a qualification/follow-up route. The practice and its authorized service providers may process this information for the purposes stated in this notice.
The direct Book Now option opens a separate patient-booking service (Archy). Its own privacy notice and terms apply.
Advertising tracking
Advertising tracking on this website is currently suspended while the practice reviews advertising-platform eligibility requirements that apply to healthcare practices. The website does not currently load advertising pixels, send their page-view beacons, or report questionnaire submissions as advertising lead events.
Previously, this website used a Meta (Facebook) advertising pixel. That pixel's configured reporting included page-view and consultation lead events; page addresses, referrer context and browser identifiers could accompany that tracking. No survey responses or contact fields were included in the website's advertising event payloads. Removing tracking does not erase information previously received by advertising platforms or other systems.
Patient questionnaire responses, treatment interests and other sensitive healthcare information are not sent to advertising platforms. Tracking will not resume merely because consent is obtained; it would require a separate eligibility review and appropriate notice, consent and withdrawal arrangements.
Other website services and cookies
The website uses hosting and security technology and website performance telemetry, which may include page paths, session identifiers, and engagement and performance measurements. Fonts are requested from Google Fonts, and images may be delivered through external asset services; these services receive network information needed to deliver resources, including IP address and browser/request information.
Observed hosting/security cookies include __dpl and __cf_bm. The questionnaire stores a session identifier in your browser's session storage. A cookie preference centre is not currently implemented; your browser controls can limit cookies, but they do not substitute for withdrawing a communication choice.
Your choices and requests
You may contact the practice at (850) 749-3394 to ask about how your questionnaire information is handled, request access or correction, request deletion where applicable, or withdraw from optional communications. Certain records may need to be retained for legal or healthcare obligations. Withdrawing a communication choice does not remove previously recorded consent evidence or automatically erase submitted information.
Security and retention
The website uses HTTPS for browser submissions and its CRM delivery. Questionnaire database tables have row-level access protections and are not directly accessible to the public. These safeguards do not guarantee security or compliance with any standard.
